Long Term Care – State of New Jersey 

Learn about pausing services if the subscriber has been admitted to a licensed Long-Term Care facility. 

What it is needed?

Documents, timeline, and notifications 

  • Official POA, Conservatorship Order or Guardianship documents, if not the customer. 
  • Completed T-Mobile Long-Term Care Facility Admission Request Form, located in the Attachment section on this page.
  • After Metro by T-Mobile receives the request, allow up to 10 business days for processing.
  • The submitter (which could be the customer) or Power of Attorney submitter may be contacted for verification purposes. 
  • For email, a reply to the email address you sent from will receive confirmation of the completed change. 
  • Before a suspend or cancellation request is completed by Metro by T-Mobile, we will attempt to contact you at the number you provide on the form.

Supported Documentation

How to authenticate the customer's need

Metro by T-Mobile may authenticate the identity of the customer or authorized representative consistent with standard account-security procedures. Authentication is used to protect account integrity but is not a condition of approving a legally valid pause or cancellation request. Supported documentation, if not the customer submitting:

  • Copy of official Power of Attorney 
  • Copy of Conservatorship Order 
  • Copy of Guardianship Documents
